- Complete mailing and/or physical address
- Phone, fax, cell and contact phone numbers
- Proof of liability coverage with a minimum of $1,000,000
- Proof of Cargo coverage with a minimum of $100,000.00
- Operating authority with a M.C. Number.
- W-9 form with a Federal tax ID number or a Social Security Number.
- Satisfactory safety rating from FMCSA.
Please Note: "D & S" Logistics must be listed as a certificate holder on your insurance certificate.
